Waiver and Release

By checking the box below, I acknowledge and agree to the following: I am the parent or legal guardian of the participant and give permission for them to attend and participate in Fred Kridler's Summer Lacrosse Speed and Skill Camp. I understand that participation in athletic activities involves risk of injury. I assume full responsibility for any and all injuries, damages, or losses that may occur during camp. I hereby waive, release, and discharge the camp organizers, coaches, and facility owners from any liability for injuries or damages arising out of participation. I certify that my child is physically fit to participate and I have disclosed any relevant medical conditions. In the event of an emergency, I authorize camp staff to secure necessary medical treatment for my child.
